Reschedule Appointment or Cancel appointment. 

Step-4: Click on the Reschedule Appointment or Cancel appointment buttons one form will be opens it shows to the resubmit or Reschedule appoinpoment locations. 

Step-5: Select our wish location and then click on submit button.
